If you have private medical insurance, look for a CUSTOMER SERVICE phone number on the back of your insurance card.  There may be specific contact information for BEHAVIORAL HEALTH or SUBSTANCE ABUSE SERVICES. 

  • Ask what SPECIFIC SERVICES are covered under your insurance, including what your DEDUCTIBLES and CO-PAYS are
  • Request a referral to IN-NETWORK PROVIDERS
  • Request that your insurance company call the TREATMENT PROVIDER directly
  • Ask if a PRIOR AUTHORIZATION from your insurance company is required for care
